Contraception Review forms

We are trialling a new service to see if we can reduce the need for you to take time from your busy lives to undergo a face-to-face review, whilst ensuring that the contraception we provide is safe and effective.

To continue a prescription we need to ensure a review is performed annually.  Only 1 months’ supply will be issued until this review is completed.

The form below can be printed, completed and returned to the practice either by post or email at stow.dispensary@nhs.net

MySpira

A new app, MySpira, has been developed by Suffolk company Orbital Media, with the help of the University of Suffolk, to help teach young asthma sufferers how to use inhalers better and help save lives.  MySpira, is a game where children have to land a rocket while learning about asthma symtoms, so they can use inhalers correctly in the future.
